The Bureau of Economic Analysis (BEA) requests your participation in the 20XX Annual Survey of Foreign
Direct Investment in the United States (BE-15), a mandatory survey that collects data from U.S.
companies in order to produce timely economic statistics.
U.S. business enterprises (including real estate held for non-personal use) are required by law to submit
this form to BEA if a foreign person (in the broad legal sense, including a company) owned or controlled
a direct or indirect voting interest of 10 percent or more of the enterprise at the end of the fiscal year
that ended in calendar year 20XX. The information collected on the survey is CONFIDENTIAL and will be
used only for statistical purposes. Please read the back of this letter for additional information about the
survey.
Due date - The due date of your report is May 31, 20XX.
New Electronic Filing Requirement
To ensure timely receipt of forms and to increase processing efficiency, BEA is now requiring electronic
submission of survey forms using BEA's secure eFile system (www.bea.gov/eFile). Paper and fax
submissions will no longer be permitted. If this is your first time using eFile, the username you establish
can be linked to the BE-15 survey using the BE-15 ID number shown at the top of the letter. Extension
requests may also be submitted via eFile. Additional information for the BE-15 survey is available at
www.bea.gov/fdi.
