Below are the instructions to complete the post-training evaluations in CDC TRAIN. Please submit your responses no later than {insert date}. The link to each evaluation can be found on your CDC Learning Plan. The results are anonymous, as we value your honest feedback. Contact Jessica Bell-Johnson if you have any questions or have issues accessing the evaluation.
The survey should take approximately 10 minutes to complete.
For the “PH LEADS 2022: The People's Approach to Improving Health and Social Outcomes” evaluation:
The course is open for registration in CDC TRAIN.
1. Log into your CDC TRAIN account. https://www.train.org/cdctrain/home
2. Type in the Leaning Plan ID {#} in the search box
3. Select launch
4. Once you launch the course, a pdf version of the agenda will pop up in a second window. Simply close the window after it opens.
5. Return to CDC TRAIN to rate the course AND complete the evaluation. (The prompt to complete the evaluation should happen immediately after you close the pdf pop-up window.)
