NEA Jazz Masters Fellowships Nomination Form

To make and complete a nomination for an NEA Jazz Masters Fellowship, you must:
1) Enter the first and last name of the individual you wish to nominate;
2) Indicate if the nomination is for the performer or advocate category;
3) Attach the two (2) required PDF supporting documents as described in the guidelines;
4) Provide the required nominator information; and
5) Click submit.
A nominee must be a living U.S. Citizen or permanent resident of the United States.
Nominations must be submitted in English.
Incomplete nominations will not be accepted.
