CDC Model Performance Evaluation Program (MPEP) for
Mycobacterium tuberculosis Drug Susceptibility Testing
Attachment 4
Online Survey Instrument Webshots
Respondents are able to submit up to two growth-based drug susceptibility testing (DST) methods and results and three molecular testing methods and results. The data collection instrument is built to include logic (a survey tool that allows a tailored question path based on respondents’ answers) to reduce burden for participating laboratories or respondents who only perform one DST method. This logic includes asking whether growth-based DST was performed (if the answer is yes, then a question is asked to determine whether a second growth-based method was performed) and whether molecular testing for drug resistance was performed (if the answer is yes, then questions are asked to determine whether a second and third molecular method was performed).
The data collection instrument webshots below show the full data collection instrument as if the respondent is submitting results for two growth-based DST methods and three molecular methods. Yellow arrows within this document showcase built-in logic to move respondents to the appropriate next question based on previously answered questions.
